Squid

Is there a way for anyone that goes through my Squid proxy to see an initial page where I can make them click Yes/No (License agreement) before I allow them to proceed?

Not that I know of, but you could use authentication in Squid and make the users sign an agreement before they get their username/password to access the proxy.
