Question hash sum error on debian 3.12.9-1 x86_64

I have a problem with my server its giving me this error message:

"W: Failed to fetch http://http.debian.net/debian/dists/.../PackagesIndex Hash Sum mismatch

E: Some index files failed to download. They have been ignored, or old ones used instead."

Itried to use the following code but still getting the same message:
apt-get clean
apt-get update
apt-get upgrade

how do i solve this problem?

i also tried :
apt-get clean
rm -rf /var/lib/apt/lists/*
rm -rf /var/lib/apt/lists/partial/*
apt-get clean
apt-get update
apt-get upgrade

and this is the message i am getting:

W: Failed to fetch http://debian.mirror.ac.za/debian/di...ssie/InRelease

W: Failed to fetch http://debian.mirror.ac.za/debian/di...ie/Release.gpg Could not resolve 'debian.mirror.ac.za'

W: Some index files failed to download. They have been ignored, or old ones used instead.

For some reason I'm not seeing your message here.

The hash sum mismatch error can happen if a mirror has bad files, or sometimes if you happen to try to update your cache while the mirror is updating its packages. Try again, and if that doesn't work, try another mirror.
