I have 2 servers running RHEL 4.7 and kernel 2.6.9-78. They host the oracle forms applications.
One day, i rebooted these 2 machines and they gave me the same error:
lvm exited abnormally!
and at the bottom: kernel panic!
I searched some and copied one of the good running server's kernel(the same: 2.6.9-78) and initrd to these machines in rescue mode. After that, everything seemed to be fine but when i rebooted again, the same error... I couldn't solve this problem and i wonder if any other machines wait for me to see themselves in a panic...
did you loss some disks e.g. from an external storage?
No i didn't... (I don't know if you use) i presented some gatekeeper disks residing on Symmetrix DMX array but these disks are not used in lvm anytime. These disks are usefull for server to communicate with the Symmetrix arrays to manage the array with symcli(symmetrix command line interface).
Otherwise, i didn't use any disk belong to external storage in this server(s).
I detached the host-bus adapters from this servers, and problems started playing...
