IntelliPoint 4.12 (5 button mouse)

I have a 5 button pS2/USB IntelliPoint 4.12 mouse. I have searched around for how to make the extra side buttons work and i gather its buy editing xorg.conf but i don't know precisely what i'm meant to do. Here is a pic of the mouse just so we all know exactly what i'm talking about:-

Here is my xorg.conf file at the moment:-
