Okay; I've installed QMMP and it works. But this doesn't help explain why XMMS doesn't work.

Still missing: reason why XMMS does not output an audio stream to Pulse Audio. Pulse Audio seems to see and handle playback audio streams from every application I've tried, but not XMMS. XMMS used to work (on this machine) until I switch to HDMI -- and switched back -- one day.

In XMMS preferences (Ctrl+P) I set the output plugin to "PulseAudio Output Plugin (libxmms-pulse.so)" and it works now. I'm certain that possibility was not available previously ... must have come in through some update.
