cross platform C or C++ API for linux and Win32

Is there an open source cross platform C or C++ API for linux and Win32?
I want to write a portable program for both linux and Win32.
How do multi-platform programs do that?
e.g. openoffice.org or mozilla, how do they design their source code so that it will run on multi-platform?

There are a number of good alternatives. I'd definitely recommend taking a look at Qt:
