iptables v1.3.5 unable to initialize table filter on CentOS 5.9

When trying to restart iptables I'm getting the following output:

/etc/init.d/iptables restart
Unloading iptables modules: [ OK ]
Applying iptables firewall rules: iptables-restore v1.3.5: iptables-restore: unable to initializetable 'filter'

Error occurred at line: 1
Try `iptables-restore -h' or 'iptables-restore --help' for more information.
[FAILED]
I've checked the filter module with the command "modprobe /lib/modules/2.6.18-348.el5/kernel/net/ipv4/netfilter/iptable_filter.ko" which returns "FATAL: Module /lib/modules/2.6.18_348.el5/kernel/net/ipv4/netfilter/iptable_filter.ko not found."

The error is odd because it's referencing "2.6.18_348.el5" (with an underscore) instead of "2.6.18-348.el5" (with a hyphen).

I tried creating a symlink of the folder name with the underscore to the one with the hyphen (the real one that exists with the modules) but saw no change.

rpm and modprobe both have funny rules about "_" vs "-". See https://github.com/saltstack/salt/issues/13239 for some description.
