binding key combination to middle click? (pasting in urxvt)

I'm trying to switch from gnome-terminal to urxvt, but having a little trouble with copying/pasting. With gterm, I always used ctrl-shift-v to paste, but that doesn't work by default in urxvt.

How can I make ctrl-shift-v appear as a middle click to Xorg?
Alternatively, if anyone knows a way to make ctrl-shift-c/v copy/paste in urxvt, that would be work too.
