Put the patch and the kernel source tree in the same directory. For example, put both in /home/hughes28105. Enter the top of the kernel source tree which would be linux-2.6 in your case. Apply the patch with the patch command. For example

patch -Np1 -i ../bcm43xx-2.6.20_combined.patch
Continue with the kernel compilation as normal.
The p1 portion of the command tells patch how many leading components to strip from the file name. You would have to look at the header in the patch file determine the correct p# to pass. If the file name begins with linux-2.6/, then 1 is the correct number.
