io error after creating


Using RHS 3.0 Kernel version 2.4 on VMWare
I tried to do create a physical volume and issued the command

1.vgscan
2. pvcreate /dev/sda1
3. pvcreate /dev/sda2

Once thse physical volumes were created, I can't seem to issue any commands like whereis or reboot. I get a 'command not found' error.
When I do a shutdown I get these type of errors:

EXT3-fs error (device sd(8,2)) in
ext3_reserve_inode_write: IO failure

Any idea what could be going wrong?

What other disks are on the system? Post the system details.
If you only have the scsi devices you did the pvcreate on you most likely are hosed
