How do I create a Xen PVH guest?

I can't find any tutorials online.

I followed Xen Project's beginner's tutorial for setting up Xen on a Debian dom0, now I'm ready to add some guests.

I must admit I've only lightly used Xen PVH, but doesn't this detail how to build a guest kernel and run it?
