patching issues

i can usually patch just fine, but every now and then on some programs i will try to patch

patch -Np1 -i patchname.patch

and get an error like

HUNK 1 of 1 failed, all rejects are kept in filename

this happens only some of the time, on other machines the patch may work just fine. What causes this? is there a dependancy i dont have? i cant give specifics as to what patches do it, as any of them may on certain machines.

It means that the patch program wasn't able to find the place modified by the patch file. There are many possible reasons. Two most common ones are:
* source you're patching is in a different version that the one the patch was build for - probably even the patch is already included
* you have another patch that changed the same fragment, so it now looks differently
