CD to ISO

Very simple question... I just can't figure it out.
I tried using dd to accomplish this, but must be messing it up some how.

All I want to do is "copy" a CD/DVD to my hard drive by creating an ISO image of the disc.
How do I do this?

If used dd to accomplish this. It's just:

dd if=/dev/hdc of=some.iso

Obviously, replace '/dev/hdc' with the device containing the media (don't mount it).
