READ CAPACITY failed


hi everyone

I have a centos6.6 on HP DL380p Gen8 that connected to EMC storage . I have recieved these errors like

or

what should i do?is it related to storage or related to my configuration?thanks in advance

Kernel version of Centos 6?

How is the EMC storage connected to the server? (bus system)

kernel version 2.6.32-504
