Proxy Server Suggestions Needed

Ok I really need suggestions for a proxy server with the following features.

1) username/password support
2) log files w/ username , ip, website, date/time
3) session timeouts / idle timeouts

Does anyone have any suggestions on non squid programs? Can anyone suggest a good newbie squid help book or site?

I would think that SQUID would do everything that you need. www.squid-cache.org is a good place to start download and read the how-tos.. very easy to get the hang of too
