Dyndns

Yes, you need to give your external IPadress to your friend to access your webserver.

If you are behind a router (or NAT) you need to setup portforwarding.

If you have a changing IPaddress (like with many Adsl providers) you can easily setup a DynDns account (http://www.dyndns.org/services/dyndns/).
Many hardwarerouters can handle dnsupdates to the dyndns site automaticly.

External (local is only for your local network, won't work outside) and yes, you'd need to do port forwarding on the firewall.

You mean a domain? You can buy one (then DNS service is often included) or get from one of the providers who give them for free (but you have less choice and longer domain names then).
