Depends on what you mean by everything. Mostly all the files, programs, applications that can be upgraded/updated are upgraded/updated. Things like your wallpaper, themes remain as they were before the upgrade.

Usually yes, unless during the checks it is found that certain settings will be changed or will be removed. You will have a chance to review the changes that will be made. You will also get the chance to not remove files selected for removal.

No you can't choose. It's an upgrade, not a clean install.
Even with the best of upgrades things can and do get broken. Ensure you have backed up any important data before starting. I have successfully upgraded using the update manager since Intrepid Ibex. Mostly it's been a smooth (but lengthy) process. I have currently upgraded to Oneric Ocelot which has produced a display issue, but then it is still in beta.
If you are not prepared to troubleshoot any potential problems I would avoid the upgrade and go for a clean install.
