FS corrupted inittab = 0

I understand that the inittab has the run levels?
and that it should never be empty. This caused a F/W corruption or is this a case of the chicken and the egg?

Where would be a good place to start to RCA.

Sorry, I'm sure I should know what "RCA" stands for but I have no idea... And what is a "F/W corruption?"

Anyway, to get better help, what distribution are you using? Are you just looking for a replacement inittab file? Was this immediately after an install or the system had been up and running for a while before it became a problem? Is there anything in the syslog?
