Don't even think about applying upgrades etc. It used to be the case you could go from the last "point" release of RHEL to the next major release but I don't believe that was the case after 5.0
Rather than migrate the server you should be looking at starting fresh with a "clean" RHEL 6.X server and migrating the applications. For that, it's a case of auditing your existing server, seeing what applications need migrating and then planning them accordingly. Then migrate and TEST, TEST, TEST before putting in to production.
Looking for steep by step Guide for migration of RHEL 4.X to RHEL 6.x . All the method using any tool or using any inbuilt command.

Download the latest version (7.x)
Put installation media in server
Format all drives, install
Restore data from backups

We are happy to help with SPECIFIC questions, but please don't ask us to look things up for you, type up entire how-to guides, etc. ESPECIALLY for commercial products. RHEL4 is ancient..RHEL6 is also old, but still supported. There *IS* no migration plan from two major versions behind...it is format/reinstall, period. Call Red Hat support.

going from 4 ( unsupported! ) to the aging 6.8 you WILL!!! need redhat's paid for support
