kernel upgrade/boot error with grub

I've just upgraded my kernel to 2.4.21 & patched for the new APCI (power management) functionality. The kernel upgrade & patch went without a hitch. I've added a grub file entry as follows:
title Red Hat Linux [ACPI Enabled] (2.4.21)
root (hd0,2)
kernel /boot/vmlinuz-2.4.21 ro root=LABEL=/
initrd /boot/initrd-2.4.21.img
Basically this is identical to all previous grub entries barring the kernel version number.
However when I try and boot with the new kernel, while everything starts ok, before long I get a kernel panic with the following error:
VFS: cannot open root device "LABEL=/" or 00:00
Please append a correct "root = " boot option
and blam! over she goes...
Any ideas here - it seems like a simple syntax error in the grub file but as I said the entry is identical to all previous entries.
The only other thing that's different is I don't have a module-info file in /boot/ for the new kernel. I'm told this doesn't matter however.
Any leads/directions before I start suspecting the APCI modules are at fault?
