I haven't used OTRS so I can't say anything good or bad about it. However, I second RT (Request Tracker). Its a pretty robust system. I took a look at the other system mentioned here and RT is not as "pretty", no icons, and frankly can be a pain to get up and running, but its rock solid.
We use it with several departments 60,000+ tickets so far. It does one thing and it does it well. Ticket tracking.
