Can't stop NVU formatting source code (even though I've disabled it)

So to illustrate, I select the option to retain original source formatting in NVU though it still formats the source file by including markup I don't want such as the head, body, etc.

http://img483.imageshack.us/my.php?image=ssdw1.jpg

What I am trying to do is implement a PHP driven template like so
Code:
<?php
include("header");
include("content);
include("footer");
?>
So I want it to be made possible to edit the content file without reformatting and adding additional markup as specified above.

Suggestions please?

Thanks
