Ok now have a full Mandrake install plus working internet, in fact typing this on Mandrake
So I guess you're using Gnome ?
Which would be
System -> Configuration -> Configure Your Computer -> Network & Internet
First I would check and remove any existing connections -> Remove a Connection
So what I did was
New connection -> LAN connection -> Chose my network interface (eth0) -> Automatic IP (BOOTP/DHCP) -> put a "x" in Network Hotplugging and Start at boot leave the other 2 boxes empty -> left Hostname blank -> left Zeroconf host name blank -> select Yes to restart the network
VIA Technologies VT82C686 (Apollo Super AC97 Modem) is nothing to do with ntl this is an internal modem on your computer

See if you can spot your modem lets see if there is anything strange about it
You do only have one NIC in your computer and this is the same one windows uses ?
From what I can work out ntl relies on the MAC address of your NIC being registered with them
If this don't work we'll go into windows and see what settings are there
