Quick and dirty:

Save this as a text file, and make it executable (chmod +x). Test it by running it as ./filename and see if it works. Park it somewhere. Run automatically by calling it from the end of /etc/rc.local. Just add a line that says "/someparkedplace/filename-of-script". Yes, it could be put somewhere in the /etc/rc3.d directory structure but this is quicker.

Honestly, I dunno. Iptables is not something I'm very good at. I always point questions like this to http://easyfwgen.morizot.net/ where you can construct your own, which has always worked very well for me.
