System is Ubuntu 14.04.1 LTS, kernel 3.13.0-43-generic. I've periodically been seeing system lockups since September. I did a bug report on this at Ubuntu LaunchPad and the response I've gotten so far is that I should update my bios or at least that's how I read it. Here is the report I submitted and the reply:
chris pollock, thank you for reporting this and helping make Ubuntu better. As per http://www.dell.com/support/home/us/en/04/product-support/product/optiPlex-780/drivers an update to your BIOS is available (A15). If you update to this following https://help.ubuntu.com/community/BIOSUpdate does it change anything? If it doesn't, could you please both specify what happened, and provide the output of the following terminal command:
sudo dmidecode -s bios-version && sudo dmidecode -s bios-release-date
For more on BIOS updates and linux, please see https://help.ubuntu.com/community/ReportingBugs#Bug_reporting_etiquette .
Please note your current BIOS is already in the Bug Description, so posting this on the old BIOS would not be helpful. As well, you don't have to create a new bug report.
Once the BIOS is updated, then please mark this report Status Confirmed.
Thank you for your understanding.

Changed in linux (Ubuntu):
importance: Undecided → Low
status: Confirmed → Incomplete
Would a bios update 'cure' this bug or is there something else entirely involved with it?
I feel they are serving you poorly, or very busy - p[robably both.
A BIOS upgrade is issued when the BIOS is screwed up in some way, and there is usually only one or two. It's rarely an annual event.
Hangcheck is probably telling you that something has gone out to lunch. It may be bios. There are boot parameters to deal with bios problems One I would suggest playing with is pcie_aspm
pcie_aspm=off #disables PCIE Active State Power Management
pcie_aspm=force # Forces the use of pcie_aspm on devices, which sometimes causes lockups.
Without these the bios gets more of a look in, if I understand this correctly.
That's quick & cheap to try, at any rate.
Apologies for the late reply, have been on vacation and of course came back to a black screen with a mouse cursor. I have found out that even though I can't get out of this except by CTRL>ALT>F1, logging in and issuing a 'sudo reboot' the system still is functioning. For instance fetchmail and procmail still poll for mail and process it. Yesterday, 18 Dec, after this happened again I updated to a newer kernel to see what happens. The odd part now is that the 'hangcheck' error has not been seen since 27 Nov so I'm not sure what was causing that. The newer issue was the black screen with the mouse cursor.
Although there have been bugs reported with the same 'hangcheck' the error also mentioned the GPU. So far the 3.14 kernel has been working fine with no issues at all. Will just have to wait and see.
I realize it's been about three months since I visited this thread. There have been no changes in the problem even though I've been going through many different kernel versions up to and including 4.0.0-040000rc4-generic #201503152135 SMP Mon Mar 16 01:36:37 UTC 2015 x86_64 x86_64 x86_64 GNU/Linux that I am currently running which still produces the error shown in the subject of this thread. For instance, yesterday morning I went to begin my morning routine, as usual the computer is on all night just running fetchmail and procmail and the other normal processes along with X-Screensaver. When I turned on the monitor on I was presented with a black screen with only a movable mouse cursor. I did a CTRL>ALT>F1 left the room and came back a few minutes later. Logged in and then ran 'sudo reboot'. The system rebooted normally. During this lockup the 'Hangcheck...' error was not noted nor was it noted on the previous one on 20 March, however on 19 March it was evident all when running the 4.0.0 kernel version. More of my comments/notations can be found here at my Ubuntu bug report https://bugs.launchpad.net/ubuntu/+s...x/+bug/1402331 I'm really getting irritated by this whole damn mess. I've never run across a system that I can't keep up for more than two days in my life and most of the time it's not even that long. I feel that the OP who is 'troubleshooting' this bug is just running me in circles. As a side note this same bug has been reported at freedesktop.org https://bugs.freedesktop.org/show_bug.cgi?id=75394 and I've also added comments to it but I don't know if it's actively being worked on and I don't know how to follow up on it.
Thanks, question is it's been reported at freedesktop.org by someone other than me but seems like it's fallen by the wayside, as far as the Ubuntu Launchpad report I'm the only one who has made mention of it and as I said I seem to be getting the 'try this, do this, well did this work and so forth' drill. Honestly he's got my head spinning with everything he wants me to try and now in one of his last comments he wants me to install and run a completely earlier version (13.10) of Ubuntu to see if the bug is there. I just can't take my 14.04 system out of service and install another one to see if it's going to screw up I have too much going on this one. Anyway, I'll check in at the Phoronix.com forums and see what I can come up with. What I'd really like is a good definition of just what really means. One more bit of information I'll add and I don't have any idea if it means anything or not but after yesterday mornings lockup I saw this in my syslog:

I never pay too much attention to what a cpu does after it pukes. It rarely leads to to why it puked in the first place.

The above line is a graphics thing, and appears pretty early in your log snippet. In monitors of old, vblank was a black level applied as the dot was in 'flyback'. It writes the line; vblank is applied; it flies back to the start; vblank is removed and signal applied; it writes the next line;
Without vblank, you see white lines where the dot traces back.

That looks like the graphics driver stopped making sense in mid line
Thanks for the reply. I went to Phoronix.com and wrote a post in the DRM forum last night I guess I have to wait now for a moderator to approve it before it can get posted. I just really wish I had a definition of what this means - drm:i915_hangcheck_elapsed] *ERROR* Hangcheck timer elapsed. I see that there is a 4.0.0-040000rc5-generic kernel our today so I guess the next time I have a lockup the guy on the Ubuntu Launchpad Bugzilla will want me to try that one to see if it happens.
HQANGCHECK timer = watchdog timer basically. It's in the kernel (Debug stuff?) and mine runs just fine without it. It sits there quiet as long it's been talked to. But when the software sticks in a loop or crashes, the conversation stops. So the software has crashed.
The difference between hangcheck & watchdog is, I think that Watchdog watches the kernel, and hangcheck can be used by any module. i915 sure uses it. Google hangcheck timer. These two look good.
Thanks for the links, I noticed these two reference pretty old versions of Ubuntu. Today an updated Ubuntu kernel was downloaded and instead of going back to the 4.0....rc4 I instead installed 3.13.0-48-generic #80-Ubuntu SMP Thu Mar 12 11:16:15 UTC 2015 x86_64 x86_64 x86_64 GNU/Linux there was a newer 4.0...rc5 out dated today, I installed and booted into that however some of my gnome tweak tools settings would not work correctly. I haven't heard anything from the guy on Ubuntu Launchpad in a few days now since I asked how I was supposed to do the work on my system that I have to do daily while regressing to a version a couple of years old.
Probably why is that a hangcheck error tells them "There has been some crash" but not what the crash was about. Have you tried disabling hangcheck? Ther you'll get a proper GPU crash which might tell you what the error is.
Google 'i915 disable hangcheck' as I did and you'll find a patch, and loads of instructions andf wheezes to do just that. Then you'll find the real error.
I don't think they are at freedesktop either from what I can see. I'm going to re-join the intel-gfx mailing list this morning and ask just what can be done or is being done about the error. I did a search on their list for 'hangcheck render ring idle' and got quite a few hits. As far as a doing a patch to disable hangcheck I sometimes find myself very unsure when it comes to doing certain things even though I've shouldn't be as many years as I've been running Linux.
Lots of people have had problems with this driver/chipset combo. I have a Brookdale 82845G here that I had very similar problems with for years. As far as I can tell, the problem is either in the inteldrmfb code, or perhaps the Xwindow/drm 915 code. Like you, I tried everything I could think of but nothing worked in a stable sense. My advise would be to either get a new video card, hold your nose and use the VESA fb console and VESA driver for X11, or get used to the command line. Support under Linux for this chipset family is just out-and-out flakey.
