My recommendation is:
(1) save all of your data that you currently have under RHEL 4. In general this means: /home, /var/www/html, /root and any changes that you know that you have made to any system files since the time that RHEL 4 was first installed. There may be other data that you want to save as well.
(2) wipe out /, /usr and /var, and then do a clean install of rhel 5.
