I get the feeling you haven't actually tried anything, just copy & pasted it at us. You paste binned this massive file which no volunteer in their right mind is going to read through. You sent us one link to 3 more links. You're not paying us and neither is anyone else. This is your system, your problem. The idea is that you do a bit of work yourself.

Haswell doesn't support Vulkan fully. What resolution & dpi are you trying to watch? Have you tried reducing it? Have you googled for similar issues? You say it's been crashing for 2-3 months. What started it? What are you doing/watching when it happens?
