that "fallen out the bus" become after i run "nvidia-smi" command - screen flashes, and sometime freeze, and no changes until hard restart. in the same time i can log in in machine via ssh - all working, dmesg show the above info. if i again from ssh run nvidia-smi command, that terminal session also lock up.
system is slackware64 14.1, x64.
drivers is latest nvidia 331.49, but previous - 331.33 shows exactly the same.
try to install fresh system on spare disc - the same.
videocard is palit GT210 /512M, PCIe
i try to swap it with my old asus 9400GT on PCIe, and not have any problems - it work about week under a load ( boinc). try to swap that gt210 on warranty, they give me new, but the same problem exactly.
motherboard is asus p8h61 rev 2.0, i3 3220 cpu, seasonic ss-430GB PSU...
any ideas?
1) Try putting it in persistent mode, see first link.
2) Try a more recent driver if available.
3) Try removing flash player.
4) You could also try the nouveau driver.
5) Could be a hardware issue if the above didn't work.

thanks for advice. ill try first.
3. cannot be relevant, because i do not use X server.
4. not work because i need CUDA and OpenCL
5. looks strange, because, as i said, that is second gt210 card with precisely the same symptom. on other side - when i replace this gt210 card with 9400gt, and no chnages anything other, all works like charm.
problem still persists with persistent driver mode too.
finally problem eliminated by changing videacard from Palit GT210 to Asus GT210.
now all works without problems with the same drivers and system.
