If you are new to MySQL, it may be worth grabbing MySQL Administrator, that allows you to create accounts and set permissions really easily.

MySQL Administrator is a really nice GUI way to manage MySQL. You can get it from http://www.mysql.com/products/administrator/. Once you have installed the program, launch it and it is easy to manage user accounts. To change permissions on an account, you simply select the account, select the database, select the permissions you would like that account to have and click a button. It is hard to explain a GUI app through text but it is so easy to use, once you have it, it won't pose any program.

Do you want to insert a username field into the table or insert a row with the username in the 'name' field?

If you just want to add a username field, Administrator can do that too. On the menu, at the bottom it says 'Catalogs', choose that, then the database that your table is in. A list of tables in that database will appear, double click on the table you wish to edit and scroll to the bottom of the new window, just type the details in there and apply it.
