Looking for software to make business forms


Subject says it all. I want to create several business forms with fields to enter names and addresses, and products bought, how many, what cost, = total

Most questions that I want to ask have a checkbox for the user to indicate 'yes' or 'no'. And I want to be able to draw boxes and rectangles, resize them, group them together, and then move them around the screen.

Sort of like what I could do in MS Access database designer screen. But I just want to print my forms (for now) for manual completion, not actually make a database or fillable form. Anyone have any ideas on what linux app might work best?

What's the linux counterpart to adobe acrobat professional, which edits and customizes pdf files. This was another idea I had.

The Writer? And have to tab around and enter spaces and use single cell tables as boxes? I didn't even think you could reposition and group objects around unless they were a part of a paragraph. I suppose it *can* be done in Writer, but if I'm going that route I'd just as use the spreadsheet.
