If Porteus 3.1 were still downloadable, I would have voted for that. As it is, I went with antiX.

My vote goes to AVLinux because of all the utilities it has.
A second runner up for me would be browser linux. I like to attempt to make older computers useful and usable by people not so familiar with linux. Idea is to make a computer as internet ready as possible with minimal clicks. I need to learn how to set up devices like usb wifi when the system doesn't automatically detect them.
I've been a little out of touch, I'll have to lookup distributions in top list to catch up.

Linux-Mint Live. My wife ran this for a week when her laptop drive died and she didn't want to buy Windows. She ran totally from the CD. I set it up for her including restoring her bookmarks from an external drive. When her new drive arrived I installed Mint from the same Live CD and she has been happy with the result ever since (about 2 months).

Slackware just pipped to the post by antiX. Congrats to both - I wasn't even aware antiX had a live edition. I haven't used it but it deserves a high place for supporting older hardware and being non-systemd.

SystemRescue lost my support when they dropped 32-bit. Luckily, there's Plop.

Actually, we have two separate things here. Rescue tools and live general-purpose distros, like Puppy or Knoppix.

Bast Live System

I voted for anTix Linux. I have never had any major issues with it.When I need a live system that just works, I always go with anTix.
