Have you installed the kernel from an rpm or something? Either install the kernel-source rpm as well or download the latest kernel from http://www.kernel.org and recompile then run the nvidia setup again.
The kernel-source isn't installed by default. You have to add it yourself.Look in the Development section with rpmdrake (or MCC) under "other". Install the source you need from there (it should be about 150mb)
OK here is your problem. You have to as root modify the xfree config file. I ofund the easiest way to do it in mandrake 10 is to go to the /etc/X11 directory as root and delete the xfree config file. Then run harddrake,(in mandrake control center) then reconfigure your graphics card. Then reboot, or if you don't feel like rebooting to a telinit 3 then telinit 5.
As an aside, I've just upgraded from Mandrake 10 Community to Mandrake 10 Official (which ended up being a reinstall for reasons I won't go into, but all my own fault).
The default NVIDIA_GLX driver that ships on the CD worked fine with Community on my PC, but not on Official.
I got the error

(EE) NVIDIA(0) : Failed to initialize the NVIDIA kernel module!
The fix was to download the driver from the NVIDIA website and install it - then everything worked fine. No idea why it should be working in Community and broken in Official - just one of those things, I guess.
