I did to config again as below:
- yum remove squirrelmail
- yum install squirrelmail
- I deleted old plugins, I install new plugin
- Grant permission: chown root:apache chpasswd; chmod 4755 chpasswd
I again see the trouble as above.

There was to need to uninstall squirrelmail. This is clearly a change_passwd plugin problem.
Amyways check the webserver error_log to see if you find something useful.
Also check the troubleshooting section of the README file and follow the instructions there.
