i'm still having the same problem.. this is in my startup log. any ideas?

I've got the same entries in my log as your first 3 and my nvidia module is working fine. Those just mean that it's the proprietary driver and not the open source nv module.
Don't know if this will help but you can try it if you want:
Boot into runlevel 3 and become root.
Uninstall the nvidia driver.
Reinstall the nvidia driver.
Type: modprobe nvidia
If everything looks good, become normal user and startx to see if it works.
Reboot into runlevel3 and become root.
Type: lsmod
If the nvidia module is listed, you should be fine. If it isn't, you can modprobe it again. I haven't used the patch and my nvidia module is working ok with my geforce 6600 and an Apple Cinema display.

what version kernel do you have? i'm using "2.6.13-15.7-default"
I had the same problem as you dunky, after installing nvidias driver the screen went black but it still worked with the nv driver. For me it turnd out to be that i had the cable to my TV inserted. After pulling it out all worked fine.
