It would appear that some of the file associations still point to kaffiene. Try KDE Control Center - KDE Components - File Associations. There are a large number of video file association you may wish to change. On my system I've un-installed kaffiene since it is redundant once kmplayer is installed.

I also have some that just show realplayer, should I change everything to kmplayer as first choice in konqueror configs as well
