QuakeLive and video settings

I can play QL fine, however when I change video settings from within the game playing full screen and click apply video settings, the screen flashes white briefly(normal for games to do this) however it then does not return me to the game.

I can hear the sounds, and I am apparantly in the game because I can fire and move...I just can't see anything. All I see is the firefox window I launched it from, however I can not interact with the firefox window in any way. Clicking anyway just fires in the game that I cannot see...

Is their any reason for this, and any way to solve it?

OK, that seemed like a one off event...I can't reproduce it anymore at least.

However by default the video settings don't seem to match what is set. If I don't change anything and just click apply setting than everything becomes smooth and anti-aliased...any idea why this would be?
