Just about any kernel you config yourself is 'custom' . It generally means that stuff that the hardware/software needs is in there, and any extra 'fluff' is not. The advice is to use any config as a starting point, but change it to suit your needs.

For example: One of my networks is a gigabit network, so my controllers on them are all 1000's. So I make sure in the kernels on them that they don't have 10 or 100 or wireless. My notebook, on the other hand, only has a 10/100 ethernet controller, and the hardware doesn't change, so I got rid of all the non-intel and nvidia,etc stuff in there.

Hope that makes it a little clearer....lol
