If the source is still on your disk try to run 'make uninstall'.
If that doesn't work, you could install the checkinstall package, included in the /extra dir on a slackware cd. Then issue the command 'checkinstall' from the mplayer source dir and a package will be created and installed over your current mplayer installation (should be the same as the former installation). That package can be uninstalled using pkgtool and since your current installation will be completely overwritten by the new installation, the files will be removed after uninstalling the package you just created.
If you don't have the source, get the source again, configure and compile it with the same options and use checkinstall. The rest will be the same as above.
