I have had similar problems.

You will need to check to ensure you have the right firmware for the driver you are using which is hot plugged on driver initialisation. Versions 0.44 and 0.45 require new firmware - http://ipw2100.sourceforge.net/firmware.php. Also check on the build of your driver it is looking in the right place for the firmware. It looks from your error messages that the driver is looking in the wrong place.

I have a dual boot system, one using the older drivers and one using the newer drivers and I have noticed that sometimes I need to do a cold boot to change drivers despite the firmware being hot plugged. I can't be bothered to work out why.
