web based Forms application
 
Just like:

Drupal does web based content management
http://drupal.org/

Gallery does web based photo album organizing
http://gallery.menalto.com/index.php

WebCalendar does web based calendaring
http://www.k5n.us/webcalendar.php

etc ...


I am looking for a Forms application that:
  • Is Open Source.
  • Is web based.
  • Runs on Linux, Apache, MySQL and PHP.
  • Actively maintained.

I need to create all kinds of custom forms.

Any suggestions?

Oracle APEX. if you use XE then it should satisfy your open source req. ( please double check their license).

I am looking at XE. This runs on Oracle only, correct?
My database requirement is MySQL.

oh, i didn't see that ( that you have to have mysql as db ). My bad. Sorry, can't help then.
