Upgrading KDE and Debian

This one is about the famous apt-get. I ran apt-get update and upgrade from the terminal, going through the installation there are instances that it asks me if I want to upgrade to a package released by the distributor or keep mine the default is keep, so thats what I do is this what I should do?
Also what is the diff between apt-get upgrade, and apt-get dist-upgrade?.......Also Im assuming that this only upgrades debian, so what is a reliable way to update KDE last time i just selected packages from the kpackage menu and it made everything really unstable.
Do i need to upgrade more than KDE? like x11 or anything else? How do I know which packages to upgrade?

I know thats alot to answer but im becoming confused, I think ill buy point and click linux now:D
