Only use package's name to uninstall. for example:
you use

rpm -ivh firefox-1.5-0.fc4.i386.rpm
to install firefox; then use
to remove it.
Beware that you may encounter dependency problem during uninstall, what distro are you using, maybe better to use a rpm manager like yum. It seems that yum lacks a "reinstall" feature like apt in debian.
and I strongly suggest you to figure out what really cause the problem before reinstalling a package. Reinstallation is the last thing to do.
