Q: Software for computer based simple questionnaires
 
Hi all,

I'm looking for a special software and haven't found any appropriate for linux, but maybe someone of you can give me a cue where to look.

I need a software to build questionnaires and perform simple calculations with the data. The questionnaires are psychological tests, where one item has several response possibilities and assigned a value each. After the input the program should at least allow me to build a sum, where I sometimes have to convert the raw data (e. g. the answer "often" sometimes gives a value of "1" and sometimes a value of "4").
Does anyone know a software to create this and to allow that the questionnaire is filled in directly at the computer? The calculation would be fine as a feature, but I think this might be possible another way, too.

I would imagine there is some PHP script out there that can do this.

Thanks for your reply. Yes, maybe I should have a look at some PHP script sites. My first google search on this reveals no good clue, but I'll dig a little bit deeper ... :)
