OIDs for harddisk partitions keeps on changing in net-snmp.

Hi,
I am using Ubuntu 8.04.3 LTS/32 bit on Intel(R) Pentium(R) D CPU 2.80GHz.

Net-SNMP version installed =>
aptitude show snmp
Package: snmp
State: installed
Automatically installed: yes
Version: 5.4.1~dfsg-4ubuntu4.2
Priority: optional
Section: net

Problem is: I am using cacti to monitor my some of the system activities including the disk reads and writes. I am using snmp OIDs to determine that.
Also I reboot my system occasionaly ( to save energy when no need ).
I found that after some reboots I cannot see the data on disk read/write graphs. Upon investigation I found that OID of the disk partitions gets change. This is internal fixed HDD.

I was using this OID .1.3.6.1.4.1.2021.13.15.1.1.12.17 for my /dev/sda3 partition to get the data to be graphed in cacti and it was working fine and gives me diskIONReadX information

But now I am getting this information for /dev/sda3 on .1.3.6.1.4.1.2021.13.15.1.1.12.21 OID (last two digits are changed)

This is very weired. Can anyone tell what exactly is the problem.

That's rather strange, have you updated your bios or net-snmp between the reboots and how is your fstab/grub referencing the drives (uuid or dev)?

Sorry , I didn't notice this thread , as I got busy with other stuff.
Grub and Fstab are referencing the partitions as UUID.
But how does it matter ? I don't think snmp has anything to do with "how grub looks at disks"?
