Is it the application that fails or am I having hardware issues?

You might have a hardware error - or somehow the disk / volume is getting mounted, and when K3B wants to eject it it is refused?

I do know with my K3B on RedHat9, K3B can override the "keep a mounted drive's door closed" thingy and eject the disk anyway.
