Icons go missing in KDE

I have a problem of missing icons. I use Mandriva 2006 Powerpack. When I use GNOME, the open as dialog in certain gtk apps like evolution looks like

In KDE also it used to look like that. But recently some icons in the dialog went missing. Now the open as dialog under KDE looks like this.

But under GNOME the open as dialog looks OK.
I changed the icon theme in KDE and changed it back to the original one. But somehoe it looks like the GTK apps have not taken back the default theme. Does anybody know where this setting is located? Can I delete some config file so that the default will be taken?

This is not a critical problem. But I just want to learn where these kinds of settings could be found.
