I have in:
/etc/xdg/xfce4/ (I use xfce4) and there is a file "xinitrc"

Open it and check, in my there is:
# Launch xscreensaver ...
if ...

You need to put # in front of everithing within "if ... fi".

Restart X after.

It depends on what is starting it.

Check ~/.xinitrc

Check /etc/acpi/events/*

Check xorg.conf

Check /etc/rc?.d/S##*

Check ~/.bash_profile

(some more likely than others)

It could also be your window manager or other things. Are you sure that it's xscreensaver and not just a DPMS setting in X blanking the screen after a certain amount of time? Well rereading the OP, the password would likely be the screen saver. But it could also be the display manager (kdm, gdm, wdm, xdm.....)
