squid authentication help

Hi, i installed proxy in my office, now it's working and i also can restrict sites, i'm little progressive in that but unable to enable authentication. In the begining blindly followed the below procedure and it

worked but for last few days it is showing error when i restart squidl. It says like "acl auth REQUIRED is wrong fatal error. authenticaion scheme is not configured properly " some thing like that. i was 100% sure i followed the same procedure what it is not working now.

Please kindly anyone show me a way to authenticate users to use proxy. Thank you very much.
