Postfix-Squirrelmail: Mail sent to user1@mydomain.com delivered to user2@mydomain.com

I've been browsing through q n as in linuxquestions.org for quite a while and the community has been rather helpful, in this case though, maybe my search strings or the problem itself, refrained me from getting an answer, so this is how it goes:

Basics:
Gentoo
Postfix
Squirrelmail

User Accounts:
User1@mydomain.com
User2@mydomain.com
User3@mydomain.com

History:
A production mailserver served working 8 months without any other issues (not as far as i know anyway)

Setup:
One forwarder implemented (from info@mydomain.com to user2@mydomain.com)
but this was done after the first couple of the following incidents.

The situation (Occasionally):

User1@mydomain.com logs on to squirellmail, selects one of her received e-mails and forwards it to user2@mydomain.com. The e-mail is delivered to user3@mydomain.com, and nothing is delivered to user2@mysomain.com.

/var/log/messages says that that the email was delivered to the intended recipient(user3). The mail header though from the users3 inbox reads mail "to: user2@mydomain.com"

I'm pretty sure that the layout of my scenario may have information missing, in this case please accept my apologies, just give me the commands you want me to run so that i can give you the right info/output.

Ok lets take it from scratch. Has anyone encountered or overheard such a behavior before? Should i say "lucky me" i just won the first prize for a "gentoo postfix reinstallation"?

what are the results for when user2 and or user3 send mails?

user 2 and 3 use Microsoft Outlook with pop3 to send an receive e-mails. User 1 is the only user using squirrelmail extensively. Actually he doesn't use his e-mail account in any other way. (Damn, i knew i had some info left out)
So in my opinion so far, squirrelmail is the only one responsible for this hick-up. I could probably set him up with a pop3 mail retrieval client and take it from there. But that's a "Microsoft way" to solve problems, where there is a problem, there is a solution around it and not for it. He's been forwarding e-mails for quite some time though, here and there he gets this behavior (something like 4-5 times in the past 8 months).

I hope it wasn't the "Microsoft" thing i said that prohibited other users from posting their insight. From the way this thread is moving (pretty fast to the second page), maybe i should consider re-setting the system. I don't have that many users that would make it to much of a pain in the a**, but still I was wondering how could this happen, or even worse, is it going to happen again!?. Never the less, is the client really sending to one user the another one receives it, or is it just a very nice prank? As far as i know, (/var/log/messages) mail is being sent to the the intended recipient. The only thing that implies that the message was sent to another user is the mail header from the forwarded message. At least, let me know if there is any other way i could possibly check that the e-mail was delivered to the wrong recipient, and not just by reading the forwarded message.
