Could be, because many mailservers do a reverse lookup for the sending IP.
You may see the logs (usually /var/log/maillog) at your end and also check the Junk/Trash folder of the recipient

The tool is to track the HTTP get and post messages but it sounds like it may of had some page requests outside of your website (possibly from a previous page load) either this or your web page pulls in external resources (such as if you have usage of Twitter feeds on your page etc etc).

It's a bit tricky to describe without a visual example so here goes: (using an old version of Apache as a simple example to demonstrate what I mean): (see attachments; I can't seem to find a way to display them within the post)

Before shows a very simple static HTML web page with an image showing no errors for HTTP status on the image

After shows when I rename the image in the file system without renaming in the HTML markup showing a 404 not found error.

Is there any of this kind of thing going on for the image in question?

Thanks, Tom. I really appreciate your efforts. But it looks like you're still addressing the issue of the captcha image not showing up. However, you'll notice in the last few posts that now the captcha image is being displayed.

But to answer your question about whether your second attachment looked like my page when the captcha was NOT being displayed, you'll notice from the code posted much earlier in this thread that the <img> tag had an "alt" text. That text is what was displayed instead of the captcha image before the captcha started working.
